🌿 Programme Overview

What Is the IMG Rural GP Programme Ireland?

The IMG Rural GP Programme is an HSE-supported, ICGP-administered initiative offering experienced international GP graduates the opportunity to work in routine daytime general practice in rural Ireland for two years. This is not a formal GP training scheme — it is a self-directed learning programme with structured mentorship, designed for doctors who are already experienced GPs and want to integrate into the Irish primary care system while working toward the MICGP qualification and entry to the Irish Medical Council Specialist Register pathway.

📋

Self-Directed Learning Framework

Unlike formal GP training, you manage your own professional development. Each participant completes a learning needs assessment at the start, maintains a reflective learning log, and works with an ICGP-assigned clinical mentor throughout the two years.

🤝

Clinical Mentorship in Your Practice

Each IMG GP is paired with a dedicated clinical mentor at their host rural practice. Your mentor provides regular oversight, guidance on Irish primary care systems, and supports your professional development — not just administrative supervision.

🎓

ICGP Educational Resources

Participants gain full access to the Irish College of GPs' extensive education library — online courses, clinical guidelines, and specialty modules covering community gynaecology, dermatology, and safer prescribing.

🏥

Out-of-Hours GP Duties

In addition to routine daytime sessions, participants take on some out-of-hours GP duties. This gives you full exposure to the breadth of Irish rural general practice — including urgent care, home visits, and after-hours patient management.

🎓 Education Programme

The Four Mandatory Education Components

This is a self-directed programme — but that does not mean unsupported. There are four mandatory components you must complete over the two years. Missing any of these jeopardises your progression to the MICGP exam pathway and the Irish Medical Council Specialist Register.

1

Learning Needs Assessment

At the start of the programme, you complete a structured learning needs assessment — identifying gaps specific to Irish general practice. You maintain a log throughout showing how you have addressed each learning need. The ICGP team will guide you on accessing educational resources.

2

Four-Day ICGP Induction Programme

A mandatory induction split into two days at the start, followed by a two-day catch-up several months later. Hotel accommodation and meals are fully provided by the ICGP — travel to the venue is at your own expense. Additional days may be required as directed by the programme team.

3

Six ICGP Online Course Modules Per Year

Each year you must complete a minimum of six modules from the ICGP online course library, relevant to your clinical practice. Eligible modules include:

  • Community Gynaecology
  • Dermatology in Practice
  • Better Safer Prescribing
4

UCD 12-Week Online General Practice Course

A structured 12-week University College Dublin online course covering topics central to Irish primary care — chronic disease management, mental health in general practice, and child health. Details of access and scheduling are communicated at programme commencement.

📝 MICGP Exam for International Doctors

The MICGP Exam Pathway — Your Route to the Specialist Register

By the end of the two-year programme, you are expected to have passed the Membership of the Irish College of GPs (MICGP) exams. ❓ Common Questions from IMG Doctors

IMG Rural GP Programme — Your Questions Answered

These are the questions we hear most often from international doctors considering IMG Rural GP Programme. Straight answers — no jargon.

Is the 3-month Paediatrics rotation strictly mandatory — or can it be substituted? +
Yes — it is a hard requirement with zero substitution. The ICGP requires a documented minimum of 3 months of postgraduate experience in both internal/general medicine and paediatrics. These are listed as separate criteria, and neither can be substituted with GP experience, emergency medicine, or any other specialty. If you have completed only one of the two, you are not yet eligible — consider gaining the missing rotation before applying. Your certificate must clearly show the specialty and duration.
Can I apply to the IMG Rural GP Programme before my IMC registration is fully complete? +
The programme requires you to submit a current IMC registration certificate as part of your application. In practice, this means you must hold at minimum a form of IMC registration at the time of applying — you cannot apply purely on the basis of a pending application. If your IMC registration is in progress, contact the ICGP programme team directly at imgruralprogramme@icgp.ie to confirm your specific situation before paying the application fee. For Pakistani doctors, we recommend reading our IMC Registration Guide first to understand the PRES exam pathway.
What actually happens during the 3-month probation period at the start of the programme? +
The first three months of the programme function as a structured probationary period. During this time, your clinical mentor at the host practice will observe and evaluate your practice style, communication with Irish patients, familiarity with Irish primary care systems, and your engagement with the self-directed learning framework. It is essentially a mutual assessment — the practice is assessing whether you are the right fit, and you are assessing whether the practice environment meets your professional needs. Concerns raised during probation can result in additional support being put in place, or in rare cases, a review of programme continuation.
How does the MICGP exam process work for IMGs on this pathway — and what if I don't pass in time? +
The MICGP exam for international doctors on this programme consists of two sequential parts: the Clinical Knowledge Test (CKT) — a 200-question paper held once per year in April — and the Clinical Competency Test (CCT), a face-to-face simulated clinical assessment held in May and November. You must pass the CKT before you can sit the CCT. The expectation is that you will have passed both by the end of your two-year programme. If you are on track with your learning needs assessment and ICGP modules from Year 1, you should be well-positioned to attempt the CKT in April of Year 1 and the CCT in either May or November of Year 2. Speak to your clinical mentor early about study leave planning — you are entitled to two weeks of study leave per year.
What medical indemnity do I need, and how much will it cost? +
Medical indemnity is a mandatory requirement under Irish Medical Council rules for all doctors practising in Ireland. You will need to obtain your own indemnity before commencing clinical work. The three main providers in the Irish market are Medisec, Medical Protection Society (MPS), and Challenge. Your clinical mentor will discuss the specific requirements with you before you start at the practice. Annual costs typically range from €2,000 to €4,000 depending on your specialty history and risk profile — budget for this separately from your programme fees.
